"他先向四周看看"

I understand this means "He looked all around". But what does 先向 mean in this sentence?

The two characters 先向 are not a single unit. They are understood individually.

先 = first

向 = toward; 向四周 = toward all around. So, it's supporting the phrase “all around."

What is the meaning of this sentence?

露在外面 = exposed outside, visible on the outside

的 = here points to what it is that is exposed

脖子 = neck

The phrase basically means, “head exposed on the outside”
